Welcome to the Responder Life Online Job Application. We use the same form for both paid staff and volunteers. Please read all instructions below carefully. If you have problems submitting electronically simply fax the entire printed form in to 503-668-8490.

Use the link below to go to the online application form, you will need to save the form to your computer. You will need Adobe PDF to use (download free here). Once you are done filling out the application, click Submit Form, it will ask to confirm your name and your email.. Make sure you see the confirmation that the form has been submitted, DO NOT CLOSE THE FORM.

Once you see the confirmation the form has been submitted then Fax only the final page of the application (EMPLOYMENT APPLICATION DISCLAIMER AND RELEASE OF INFORMATION AUTHORIZATION) to 503-668-8490. If you have already closed the form simply open it, or download a new form, and print the final page, sign and fax. Once faxed you may close the form.
